Abstract:

The clinical environment is dynamic, necessitating innovative approaches to deliver education and validate ongoing competencies. The Donna Wright Competency Assessment Model is a framework that promotes ownership, empowerment, and accountability, enabling departments to determine their competency needs. Cardiac arrest response was identified as an annual competency needed in the critical care unit. While traditional Simulation-Based Education (SBE) is an effective tool for teaching emergency response, Rapid Cycle Deliberate Practice (RCDP) was selected to meet this need due to the unique focus on microskills, repetition, and real-time feedback.

Learners participated in one-hour session using RCDP using a predetermined cardiac arrest algorithm as a means for validating competency and increasing nurse confidence in cardiac resuscitation response. Facilitators identified preselected micro skills as potential stopping points for feedback; however, participant performance determined the learning opportunities provided. Skills were solidified by running one final scenario without interruption, followed by a debrief.

Pre and post-simulation surveys were collected and analyzed utilizing the 5-point Likert scale. Survey questions assessed the participant’s confidence in performing the following aspects of resuscitation: initiation of a code response, assuming a team lead role, utilization of the cardiac arrest algorithm, use of end-tidal carbon dioxide (ETCO2) to measure the effectiveness of compressions, and return of spontaneous circulation (ROSC).

Results showed participant confidence increased in all areas measured and the RCDP methodology allowed skill development outside of those initially identified and included teamwork, collaboration, delegation, and creative thinking.

Learning Objectives:

  1. Illustrate the use of the Donna Wright Competency Assessment Model for the identification of annual competency needs in the complex clinical environment.
  2. Construct a creative method for validating identified annual competencies using rapid cycle deliberate practice (RCDP simulation).
  3. Evaluate the benefits of real-time feedback, repetition, and micro-skill mastery in medical simulation training.
