MilExTS® (Military Extended Reality Trauma Simulator) is a next-generation Extended Reality (XR) simulation platform designed to train military medics, combat lifesavers, and civilian emergency responders in Tactical Combat Casualty Care (TCCC). Aligned with U.S. Department of Defense standards and NATO training frameworks, MilExTS® replicates the urgency, stress, and complexity of trauma care in high-risk environments—without relying on live actors, manikins, or constant instructor oversight.

Built for standalone XR headsets like the Meta Quest 3, MilExTS® provides a portable and cost-effective training solution that can be deployed anywhere, including disconnected or austere environments. Trainees engage with over 450 interactive trauma scenarios grounded in real-world cases, including point-of-injury, Role 1 care, and multi-system trauma management. These scenarios are powered by a real-time physiology engine that simulates dynamic casualty responses to interventions, enhancing realism and clinical accuracy.

MilExTS® supports both single-player (no Authority to Operate required) and multiplayer (ATO required) training modes in Virtual Reality (VR) and Mixed Reality (MR). It offers guided and unguided training paths, performance analytics, and RAPTOR integration for tracking decision-making, timing, and patient outcomes. Up to 100 offline sessions can be conducted per device annually, making it ideal for field training without network access.

The platform is interoperable with Learning Management Systems (LMS), physical manikins, and Live-Virtual-Constructive (LVC) systems, enabling seamless integration into existing training ecosystems. Custom scenario development and localization capabilities allow organizations to tailor training to specific mission sets, geographic regions, and medical protocols.

Validated in field conditions—including by Ukrainian medics on the battlefield—MilExTS® has demonstrated operational impact by improving readiness, reducing costs (as low as ~$1.48 per trainee), and increasing survivability in combat and crisis scenarios.
